You would get what is in the picture, the description refers to the connectors

So IC5 battery connect to IC3 device connector

Will this convert an IC3 battery plug to an IC5 connection? I have 2 3S IC3 batteries that I want to run as 6S in a Mojave which is a IC5 connection

That lead will convert a EC5 battery to EC3.
